ABSTRACT

Objective To explore the value of Logistic regression model in evaluation on automated breast volume scanner (ABVS) combined with conventional ultrasound in differential diagnosis of breast imaging reporting and data system(BI-RADS) 3-5 category breast nodules. Methods Totally 216 patients (247 BI-RADS 3-5 category breast nodules) underwent ABVS and conventional ultrasound.Taking puncture biopsy or postoperative pathology as the gold standards,Logistic regression model was established and main ultrasound characteristics in diagnosis of breast malignant nodules of BI-RADS 3 5 category were screened and evaluated. Finally,ROC curve of evaluating the diagnostic efficiency of Logistic regression model was drawn. Results There were 4 independent variables in diagnosis of breast malignant nodules of BI-RADS 3 5 category, including " Retraction phenomenon" (X1,odds ratio [OR] =12.03,P< 0.01)," microlobulated margin" (X2,OR-6.00,P<0.01),border (X6,OR=11.48,P=0.01) and anteroposterior to transverse ratio (A/T,X8,OR=4.09,P=0.01).OR of "retraction phenomenon" was the highest. The regression equation was Logit(P) =-4.43+2.49X1 + 1.79X2 +2.44X6 +1.41X8 (x2 =196.32,P<0.01),and the area under the ROC curve was 0.95 (P< 0.01).Conclusion Logistic regression model has great value in differential diagnosis of breast malignant nodules of BI-RADS 3-5 category.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the differences in the clinical therapeutic effects on early age-related macular degeneration (AMD) in the treatment betweenacupuncture technique and regular acupuncture.</p><p><b>METHODS</b>A total of 110 patients of AMD were randomized into an observation group (55 cases, 73 eyes) and a control group (55 cases, 76 eyes). In the observation group, acupuncture was applied at Cuanzhu (BL 2) and Yiming (EX-HN 14). The needles were retained for 30 min. Additionally, Ganshu (BL 18), Pishu (BL 20) and Shenshu (BL 23) were stimulated with quick needling technique.needling technique was applied. The needle was rotated with the thumb and index finger of the puncture hand, forward and backward. The strong and large amplitude were given when the thumb moved forward, and while, the week and small amplitude required when moving backward. The gentle movement was required in the entire procedure. In the control group, the point selection was same as the observation group. The points were stimulated with regular technique. The needle retaining time was same as the observation group. The treatment was given once every two days in the two groups and for 3 months continuously. The visit was followed up for 3 months. Before and after treatment, as well as at follow-up, the eyesight, the effect on macular area and the macular retinal structure [the macular nerve fiber layer (MNFL), retinal neurepithelium layer (RNL), pigment epithelium and choroid capillary composite lay (PECCL)] were evaluated separately.</p><p><b>RESULTS</b>1. After treatment and at follow-up, the eyesight was improved as compared with that before treatment in the two groups, indicating the significant difference (both<0.05). The differences in comparison before and after treatment, and in comparison of the follow-up with that before treatment, were improved in a larger amplitude in the observation group as compared with the control group, indicating the significant difference (both<0.05). 2. Regarding the therapeutic effects on macular area, after treatment and at follow-up, the total effective rates were 83.6% (61/73) and 86.3% (63/73) in the observation group and were 63.2% (48/76) and 67.1% (51/76) in the control group separately. The results in the observation group were higher than those in the control group, indicating the significant difference between the two groups (both<0.05). 3. Regarding the macular retinal structure, after treatment and at follow-up, the levels of MNFL, RNL and PECCL were all reduced as compared with those before treatment, indicating the significant difference (all<0.05). After treatment and at follow-up, the levels of MNFL, RNL and PECCL in the observation group were all lower than those in the control group, indicating the significant difference (all<0.05).</p><p><b>CONCLUSION</b>Theacupuncture technique achieves better clinical therapeutic effects on treatment of early AMD as compared with the regular acupuncture.</p>

ABSTRACT

Objective To explore the prevalence of age-related eye diseases (AREDs) among people aged 70 years or above in Hangzhou and evaluate the impact of AREDs on visual function (VF) and visual-related quality of life (QOL) in elders. Methods This study involved a total 2 111 elderly people (≥70 years). All participants received visual acuity and comprehensive eye examinations and complete VF and QoL questionnaires. Results The main cause of visual impairment was AREDs.And the causes were age-related cataract (79.82%),AMD (45.64%),glaucoma (10.95%) and diabetic retinopathy (DR,7.30%).VF and QoL scores declined gradually with age.And the scores declined more rapidly with declined visual acuity among the elders.VF and QoL scores in patients with age-related cataract,AMD,glaucoma and DR were successively lower. After adjusting for age, gender and visual acuity, the elders with AREDs had lower scores across all domains of VF and QoL.Scores for subscales of VF and QoL domains were significantly lower among those with DR and glaucoma compared with those with age-related cataract and AMD.Conclusion Age,presenting vision and AREDs are associated with VF and QoL in this elderly population. And senile people with glaucoma and DR have severe declines in VF and QoL,independent of presenting visual acuity.

ABSTRACT

Objective To assess the value of two-dimensional and color Doppler ultrasonography in diagnosis of Hashimoto thyroiditis (HT) .Methods Sixty-five patients with HT (HT group) confirmed histologically underwent two-dimensional and color Doppler ultrasonography.Sixty volunteers with normal thyroid ultrasonogram were enrolled as control group.Ultrasonic characteristics (size, echo, blood flow distribution, peak systolic velocity of the superior thyroid artery, accompanied nodules) ,pathological finding and laboratory parameters were recorded. Results The volume of thyroid increased in HT group than that of control group (t=10.748,P<0.001) .Ultrasonic characteristics of HT were as followed:hypoechoic parenchyma (51/65,78.46%) ,multiple liner bright echoes throughout the parenchyma (46/65,70.77%) ,single or multiple macronodules in the parenchyma (6/65,9.23%) .Color Doppler flow imaging (CDFI) showed obviously increased blood flow (36/65,55.38%) ,slightly increased blood flow (25/65,38.46%) ,decreased or normal blood flow (4/65,6.15%) ,among which statistical differences of T3,T4,TSH,V_(max) were detected (P<0.001) .Conclusion There is close relationship among ultrasonic characteristics.pathological and laboratory parameters of HT.Ultrasonography has high value in diagnosis and differential diagnosis of HT.
